New Patient Registration
During the month of December 2021 and January 2022 we will not be accepting new registrations from patients within neighbouring Practices. If you are new to the area, to join the practice please speak to a member of the reception staff who will take details which will enable us to further your request for registration. All requests to register are considered by the GP partners. The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition but if a resident lives outside the practice area, registration may not be agreed.
There are updated forms which are needed to for you to register. These forms will state what additional information is required to progress your registration. All patients will be asked for photographic ID and proof of address and you must register in person.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
